Midline serrated lesions (MSLs) are traditionally described as a subtype of serrated polyps. However, the differential diagnosis of these lesions from true serrated polyps is unclear. The aim of this study was to evaluate the clinical and molecular characteristics of these lesions. A total of 1256 colonic lesions with adequate follow-up data were collected from 773 patients. Of the 1256 colonic lesions, 30 had MSL. Clinicopathological features, including BRAF and KRAS mutation status and CDX2 expression, were assessed for each patient. Twelve of the 30 MSLs were subsequently resected and were included in the molecular study. The median follow-up period was 54 months (range: 12-120 months). Most patients were women (75%) and median age was 71 years (range: 35-93 years). All 30 lesions were located in the rectosigmoid area and 12 (40%) were polypoid. Histologically, 8 (27%) were traditional serrated adenomas, whereas 22 ( eea19f52d2
